My cat rattles

Something has been irritating Princess for quite some time, but it seems to have localised to her right ear. I keep an eye on the scratches and apply antiseptic occasionally, but this afternoon she presented with a new symptom.
When she shakes her head, something rattles. She's not the brightest of cats but I don't think it's her brain bouncing around. It's also not the batwing sound any cat makes if they shake their head. My best guess is that a scab has fallen down the ear canal, but the sound might be coming from air bubbles too. Maybe she has a sinus infection? Off to the vet this evening.
She was about due for some kind of consultation because we've reached the point where the hypoallergenic diet would have cured all her ills if it was ever going to. In best vague chronic illness style it's helped a bit, but it's not a cure. I'm not sure the flea treatment made any difference.
She has lost a bit of weight, but I can't tell if she's currently losing more or has stabilised from the initial shock of the change in routine. Poor thing really does look like an old cat these days, and she's a mere 14(ish) years old.
ED: Back from the vet and there's definitely a lot of wax that won't come out without sedation (a nice bottle of scotch for the vet). The rattling is unusual but not unheard of - it's probably a ball of dried wax hanging on by a hair. She goes in tomorrow morning.
