How is electrical stimulation not for you? Because there are some vibrating handhelds that may be good. (Even those ahem, 'personal massagers' can actually be very good at loosening hard muscle knots, like the ones under the shoulderblades.)

Perhaps warming gel packs? Or poultice type things? How are you with heating creams?
